Your three-month program includes:

Cost: $500 every 3 months

Prescription treatment is provided only when clinically appropriate following consultation, evaluation, and any required laboratory testing. Additional testing or services may carry separate fees.

Using Insurance for Medication Coverage?

For patients who are eligible based on laboratory workup (for example, testosterone levels out of range low), insurance may be used for the prescription, and the patient will be charged $75 per office visit. Medication, laboratory testing, and other services may be billed separately by the applicable pharmacy, laboratory, or insurance provider. Coverage and out-of-pocket costs vary by plan.

How long does the medication supply last?
